Sports Screening Required Forms

​All of our organization's staff and medical professionals generously donate their time to help us achieve our mission and vision.  Our physical begins with the basics of blood pressure, heart rate, vision, teeth and skin examinations and culminates with extensive heart, lung and abdomen screenings.

Musculoskeletal exams are used to detect any bone or joint problems and the onset of scoliosis.  In addition, EKG stations are available on-site to complete an immediate EKG on any child found to have an abnormal heart rhythm or murmur. Our onsite cardiologist can read the EKG and provide a treatment plan to the parent in the event further care is needed.
